The California Partnership for Long-Term Care (Partnership) strives to make sure our agents are well informed of all new changes with the Partnership. In order to accomplish this task, we need your help. We would like for you to complete and submit an Agent Database Entry form. If you are an agent with no new changes, please disregard this message.
Please provide any current agent changes such as (name, home address, phone numbers, email or faxes).
There are two options available to locate the agent database entry form:
1. Download the form, print and fax or mail it to the Partnership; or
2. Complete the form online.
The Partnership is unable to continue to provide services to licensed agents, without accurate or current information. Please help us serve you.
Also, as a reminder, here are the requirements for a Partnership authorized agent: Any agent participating as a licensed Agent selling or marketing California Partnership for Long-Term Care policies shall adhere to both requirements listed below, before selling or marketing the Partnership product:
1. Complete eight (8) hours of education on long term-term care in general that meet the requirements of and
2. Complete eight (8) hours of training in a live classroom setting on the California Partnership for Long –Term Care
